Until we have more investigations to validate the results of the PAVED study related to more aggressive dosing of vancomycin, pediatric patients should still be prescribed a vancomycin dose of 15 mg/kg/dose every 6 hours. According to the Antibiotic Dosing Guideline for Cystic Fibrosis, the recommended vancomycin dosage for pediatric patients is 15 mg/kg/dose every 6 h .
